Rates:
Individual Therapy …..$250 per session
Couples Therapy …….$350 per session
Disability Forms ………$300 for initial evaluation session
Full payment is expected on the day of service. SOLA does not accept insurance but will happily connect you with the company Thrizer who will help seek partial or full reimbursement from your insurance company if you have Out of Network Benefits.
I am a contracted provider with the employee assistance program (EAP), Lyra Health. If you have EAP sessions through Lyra Health then you can use those sessions to see me without any cost to you. Contact your employer and Lyra Health to find out how many EAP sessions you have per year and let me know that you will be using these benefits during our consultation.
